Who is responsible for your information

Willerby Dental Care Ltd is the controller of the personal information used for your evaluation.

Information we collect

The website collects your name, email address, mobile number, date of birth, optional gender, TAN evaluation reference and acknowledgement records. If you separately choose to answer the two optional background questions, it also records that consent and your fixed-choice answers.

Optional background questions

The background questions are optional conversation prompts. They are not scored and do not affect allocation or access to the evaluation.

Secure access and service providers

Evaluation responses are stored in the private evaluation system in Supabase and are available only to explicitly approved practice reviewers. Sanity authenticates those reviewers but does not store the responses. Vercel hosts the website. Resend sends the customer confirmation and a notification-only practice email; notification emails contain no name, contact details, TAN reference, date of birth, gender, consent value or background answers.

Retention and withdrawal

Date of birth, gender, background consent and background answers are erased automatically after 12 months, or earlier following an accepted withdrawal request. The core TAN enrolment, allocation, review status and metadata-only access history are retained under the separately approved practice retention schedule.

Lawful basis and your rights

The lawful bases, any special-category condition, rights handling and DPIA decision must be approved before production enrolment opens. Call Willerby Dental Care on 01482 656656 with a question or withdrawal request about your evaluation information.
